Health Assessments for a variety of patients
A comprehensive annual health check is recommended for all people aged over 75 to identify any health issues and optimise quality of life. Areas assessed may include but are not limited to lifestyle, mobility, medication, nutrition, exercise, safety in the home, care arrangements and support networks.
The health assessments are not limited to people aged 75+ but also include those aged 45-49 with a risk for chronic disease; Aboriginal or Torres Strait Islanders; 40-49 year olds with high risk for diabetes; people with an intellectual disability. A number of our Registered Nurses and Allied Health Professionals can work with you in conjunction with your GP to conduct a health assessment to identify any health risks or other care needs and organise any follow-up care required.
Lifestyle Psychology- Onsite
Zebunnissa Khan is the Principal Psychologist at Lifestyle Psychology. Zebunnissa has been working in the area of mental health for over 15 years in Sydney, South Coast and is now based in Canberra.
She helps her clients, who include children, adults and families to find healthy perceptions of themselves.
